The Home

New Pathways Living Services provides a six-bed, high-support residential service for adults with complex mental health needs who require a structured environment following inpatient care or who need additional support to maintain stability in the community.

The service has been developed to address the gap between hospital discharge and independent or supported living, providing a structured, staffed environment where individuals can stabilise, rebuild daily functioning, and progress safely towards independence.

OUR RECOVERY PATHWAY

We support adults aged 18-65 who are clinically ready for discharge from inpatient services but require a step-down placement, as well as individuals who have experienced repeated hospital admissions or are not suitable for standard supported living due to the complexity of their needs.

Our approach focuses on stabilisation, structured support, and progression. Individuals are supported to regain confidence, develop daily living skills, and engage with their recovery within a safe and predictable environment.

The service operates a 24-hour staffing model and works closely with Community Mental Health Teams, social workers, GPs and other professionals to ensure coordinated care and safe progression.

Key outcomes of the service include supporting safe step-down from inpatient care, reducing the risk of readmission, improving independence, and enabling planned transition into longer-term accommodation.

Residents are supported within a shared residential setting with private bedrooms, communal living spaces, and a structured daily environment designed to promote stability and recovery. The service follows a clear pathway of stabilisation, rebuilding and progression, with placements typically time-limited and focused on achieving measurable outcomes.
