High Support Transitional Placements
New Pathways Living Services provides a high support transitional residential placement for adults with complex mental health needs.
The service is designed for individuals who require a period of structured support following inpatient care or where previous community placements have not been successful.
Support is provided within a 24 hour staffed environment and is focused on stabilisation, recovery and progression towards independence.
Daily Living
Residents receive structured support with routines, organisation and independent living skills.
Emotional Wellbeing
Consistent staff support helps monitor mental health stability and identify changes early.
Medication Support
Medication prompting and administration support is provided where required alongside professional liaison.
Promoting Independence and Stability
Residents are supported to engage with community resources, attend appointments and gradually reintroduce activities that promote independence and social inclusion. Positive Behaviour Support is used to understand behaviour, identify triggers and reduce escalation through proactive and supportive approaches.
Collaborative and Safe Care Planning
Risk is managed through individual risk assessments and clearly defined management plans, which are reviewed regularly.
The service works closely with Community Mental Health Teams, social workers, GPs and other professionals to ensure coordinated care.
Placements are typically time limited, usually between three and twelve months, with clear progression planning from the outset.
